Catastrophic: No Alternatives in place. Continued safe operations are put into immediate danger. Defect has potential to directly cause an aircraft hull loss and/or causing multiple fatalities.

Hazardous: No Alternatives in place. Can cause an immediate threat to aircraft safety. Defect requires emergency measures by the flight or ground crew to maintain aircraft safety and/or safety of human life.

Major: Some Alternatives in place. Safe and reliable operations impacted. Defect requires non- standard corrections to be taken by flight or ground crew.

Minor: Alternatives in place. No operational or safety-of- flight affected; but reliability is impacted. Defect requires non-standard physical correction by ground crew or extensive trouble- shooting to restore.

Negligible: Alternatives in place. No operational or safety-of- flight affected. May require deviation.

Catastrophic: Loss of aircraft and or multiple fatalities.

Hazardous: State of emergency impacting the immediate safe operation of an aircraft. Emergency actions by air crew required to preserve aircraft integrity and/or life.

Major: Deviation impacting the continued safe operation of an aircraft.

Minor: Deviation with some potential to affect safe operation of an aircraft. Managed by air crew using non normal checklist procedures.

Negligible: Deviation with minimal potential to affect the safe operation of an aircraft. Managed by air crew using normal procedures.

Catastrophic: Multiple fatalities.

Hazardous: One fatality admission to hospital as an inpatient total disability.

Major: Immediate admission to hospital as an inpatient and/ or permanent partial disability.

Minor: Treatment by a registered medical practitioner. Ongoing treatment required, no permanent disability.

Negligible: First aid or treatment by a certified first aider and/ or medical practitioner. No follow up required.

Catastrophic: Extended negative international media coverage or internet activity resulting in a damaging change in public and/or influential stakeholder’s perceptions for a sustained period.

Hazardous: Negative international media coverage or internet activity resulting in a major change in public and/ or influential stakeholder’s perceptions.

Major: Negative international media coverage and/or internet activity resulting in minor changes in public and/or influential stakeholder’s perceptions.

Minor: Extended negative media coverage and/or internet activity with no change in public and/or influential stakeholder’s perceptions.

Negligible: Short term negative media attention and/ or internet activity with no change in public and/or influential stakeholder’s perceptions.

Catastrophic: More than 2000 guests: Experience disrupted >48 hours. Less than 2000 guests: Experience disrupted >72 hours.

Hazardous: More than 2000 guests: Experience disrupted 24-48 hours. Less than 2000 guests: Experience Disrupted 48-72 hours.

Major: More than 2000 guests: Experience disrupted 7-24 hours. Less than 2000 guests: Experience Disrupted 24-48 hours.

Minor: More than 2000 guests: Experience disrupted 3-7 hours. Less than 2000 guests: Experience Disrupted 7-24 hours.

Negligible: More than 2000 guests: Experience disrupted for up to 3 hours. Less than 2000 guests: Experience Disrupted 3-7 hours.

Catastrophic: Station or route operations ceased/ suspended for over 24 hours.

Hazardous: Station or route operations disrupted/ suspended for 24 hours.

Major: Station or route operations disrupted/ suspended for 6-12 hours.

Minor: Station or route operations disrupted/ suspended for 3-6 hours.

Negligible: Station or route operations disrupted/ suspended for up to 3 hours.

Catastrophic: Very serious environment an effect with destruction of ecosystem. Long term, widespread effects the environment.

Hazardous: Serious effect on the environment (air, water, soil etc.). Relatively widespread with medium- long term implications.

Major: Moderate effect on the natural environment (air, water, soil etc.). Does not affect ecosystem function. Short- medium term implications.

Minor: Minor effect on the natural environment (air, water, soil etc.). Short term damage to a small area of low implications.

Negligible: Slight damage to an area of low implications.

Catastrophic: Major structural failure of, or damage to, multiple physical assets making it unusable for operations.

Hazardous: Heavy damage to physical assets which can be recovered/ repaired in more than 3 weeks.

Major: Heavy damage to physical assets which can be recovered/ repaired within 1-3 weeks.

Minor: Damage to physical assets which can be recovered/ repaired within 1-7 days.

Negligible: Damage to physical assets which can be recovered/ repaired within 1 day.

Catastrophic: Significant litigation or non- compliance issue that could disrupt operations, e.g. prosecution, injunction, material fines, etc.

Hazardous: Significant litigation or non- compliance issue that could disrupt operations, e.g. prosecution, injunction, material fines, etc.

Major: Major litigation or non- compliance issues with heavy fines from the Regulators.

Minor: Litigation or non- compliance issues with possible of minimum fine.

Negligible: Litigation or non- compliance issues that has no litigation consequence.
